This policy explains what this website collects. Information you provide during an actual visit or appointment is handled separately under our clinical privacy practices, which are governed by HIPAA and provided to you at the practice.
When you use our contact form, we collect the information you type into it: your name, email address, phone number (optional), which location you are contacting, the general topic you select, and the contents of your message. You choose what to put in the message field — please keep it non-medical.
When you book an appointment, our Schedule page sends you to EyeCloudPro, our practice management provider. Any information you enter there is collected by EyeCloudPro under their own terms and privacy practices, not by this website.
When you simply browse, we collect standard analytics: pages viewed, approximate location (city-level), device and browser type, how you arrived at the site, and how long you stayed. This is collected through Google Analytics 4 using cookies and similar technologies. It is not linked to your name and we do not use it to identify individual patients.
We do not sell your information. We do not run advertising pixels or share data with advertising networks on this site.
We share information with these providers only so they can perform these functions for us. We do not share your information with anyone else except where required by law.
Contact form messages are retained in our email for as long as needed to respond to you and maintain a record of the conversation. Analytics data is retained according to Google Analytics’ standard retention period. You can ask us to delete your contact form submission at any time.
Illinois residents, and residents of states with comprehensive privacy laws, may have additional rights regarding access, correction, deletion, and portability. Contact us using the details below and we will honor applicable requests.
This website is not directed to children, and we do not knowingly collect information from children through it. We do of course care for pediatric patients — that information is collected at the practice with a parent or guardian, under our clinical privacy practices.
This site is served over HTTPS, and administrative access to our content system is restricted to authorized staff with individual accounts. No website can be guaranteed perfectly secure, which is the reason for the notice at the top of this page: please keep health information out of web forms.
